ABOUT THE
CARD
BYID Cards are issued:
- strictly to those between the
ages of 19 and 35
- to confirm the bearer has attained
the age of 19 years
- solely for the purpose of purchasing
beverage alcohol in Ontario
This form is available for viewing
online using Adobe Acrobat Portable
Document Format (PDF). It contains the
same graphics, typography, and layout
as the printed version. You must have
the Adobe Acrobat Reader installed on
your computer to view this report. If
you do not have the Adobe Acrobat
reader application installed on your
computer, you can download it free
from the Adobe
Acrobat Web Page.
Please complete the entire application
and ensure all required documentation,
including payment, is enclosed. Failure
to do so will result in your application
being returned. For information
about your application, please call
416 864-6632. For general inquiries
about the BYID Card, please call 1-800-ONT-LCBO (1-800-668-5226) or 416 365-5900 in
the Toronto area. The TTY numbers
for the deaf and hearing impaired

Please allow four weeks for processing
the application and mailing the BYID
Card.
INSTRUCTIONS:

1: |
Complete and sign
Section 1 of the Application form. |
| Step
2: |
Enclose a copy
of one of the following
documents showing proof of name
and date of birth. Each document
must be signed by the Guarantor:

• Birth Certificate
• Canadian Citizenship
Card
• Notarized affidavit
showing name and date of birth
• Valid Passport
Note: If the applicant’s
name varies from the
proof of age document, include
proof of name change, also signed
by the Guarantor. Acceptable
documentation includes a copy
of a marriage certificate,
or if applicable, adoption
papers.

| Payment: |
A certified cheque
or money order, in the amount
of $20, made payable to the
LCBO. |
| Photograph: |
One clear colour
passport photo (50 mm x 70
mm), taken within the last three
months, showing applicant's head
and shoulders. The Guarantor
must sign the back of the photograph
verifying the photograph to
be a true likeness of the applicant. |

3: |
Obtain the Guarantor’s
certification and signature in
the Guarantor’s Certification
section of the form (Section 2).
